Welcome to the Moore Lane Veterinary Hospital Family!

At Moore Lane Veterinary Hospital, our mission is to treat our clients as we would our families and treat them as though they are our own. When you arrive, you will be greeted by our reception staff, and they will get you started with either a new client form or if you filled it out ahead of time, we’ll have you finish that up with a quick signature. Then we will let your technician know that you are ready to be seen.

Your technician will take you to your exam room, where they will gather history and get to know you and your pet. After your technician reports back to your doctor, they will then come in and examine your pet. Once your pet is all ready to go and taken care of, you’ll head back up front, and a receptionist will check you out and schedule you for your next appointment.

Payment Options

We accept Discover, Master Card, and Visa, American Express, and cash. We do not provide payment plans, however, we do accept CareCredit and ScratchPay.

Highlights of the CareCredit program:

When unexpected illness strikes a pet, unexpected expenses strike as well. Temple Heights Animal Hospital understands this and is able to make some special arrangements through the CareCredit program. It takes just 5 minutes to complete an application and will allow you to break down your payment into 6 monthly installments if you spend more than $200. If your balance is paid in 6 months, it’s interest-free. You can either apply at our hospital or apply yourself over the phone (800) 365-8295 or online by clicking the button below.

Please complete this from as fully as possible prior to your first appointment which will help expedite the registration process and give us valuable insight in providing optimal care for your pet(s).

Payment is required at the time of service. Moore Lane Veterinary Hospital accepts VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS, CARE CREDIT, and SCRATCH PAY as methods of payment. We are unable to accept checks.
I understand that every effort will be made to achieve a successful outcome and to provide safety in hospital care and handling. I hereby authorize this hospital to receive, prescribe for, treat, or perform surgery upon the pet(s) listed on the reverse side and additional pets I present.
I agree to pay fees for services rendered at the time the pet is discharged from the hospital or the service is otherwise terminated. By signing below, I acknowledge that I will be responsible for any interest and collect fees should my account be referred to a collection agency due to non-payment. I also understand that if legal action is required to collect any unpaid balance that I may be held responsible for any reasonable attorney's fees that may be uncured. I understand that a service fee of $30.00 will be assessed for each non-sufficient fund check and/or certified letter that must be sent.
If I neglect to pick up my pet within 5 days of the discharge date and do not contact Moore Lane Veterinary Hospital to make alternative arrangements, my pet will be considered abandoned, at which Moore Lane Veterinary Hospital will re-home the pet. I will still be financially liable for cost incurred prior to the date the animal is considered abandon.
